9.5. The costs exigible from a physician who submits to the Board his claim for payment or his statement of fees by means of a printed form shall be $0.50 per claim for payment or statement of fees submitted where he is remunerated per consultation for the insured services for which he is claiming payment.
Notwithstanding the foregoing, no costs are exigible
(1)  where the claim for payment or statement of fees is accompanied by the documents required by the Board and needed by it to justify the payment claimed;
(2)  where the claim for payment or statement of fees is submitted to claim payment of an insured service in respect of which a claim for payment has already been made and refused by the Board, but that may be claimed again; or
(3)  where the claim for payment or statement of fees in respect of which the insured person’s identification is not required by the Board concerns an insured laboratory service furnished in a centre operated by an institution.
No costs are exigible from a physician who, on 19 November 1995, has held for at least 30 years a permit authorizing him to practise his profession.
In the case of a pharmacy owned by several persons, the exemption provided for in the third paragraph applies only if all the pharmacists owing the pharmacy meet the condition provided for therein.
